Informed Consent Documents and Other Management Forms
The American Association of Orthodontists offers a number of informed consent documents, developed by the AAO Council on Orthodontic Practice in conjunction with the AAOIC. These are living documents which are reviewed periodically to keep them current. All AAOIC insureds are required to have a completed informed document for every patient.
Please keep in mind that informed consent is doctor-specific. Whenever a new doctor treats a patient, informed consent should be obtained.
