Informed Consent Documents and Other Management Forms
AAOIC offers a number of informed consent documents, developed by AAOIC, some in cooperation with the AAO Council on Orthodontic Practice. These are living documents which are reviewed periodically to keep them current. All AAOIC insureds are required to have a completed informed document for every patient.
Please keep in mind that informed consent is doctor-specific. Whenever a new doctor treats a patient, informed consent should be obtained.
